Allegation

As a registered Paramedic (PA07397):  
1. On or about 22 February 2021, in relation to Service User A (“Patient A”), 
you:  
a. Did not undertake an adequate and/or timely primary and/or 
secondary survey;  
b. Did not roll Patient A onto their back;  
c. Did not undertake adequate clinical observations;  
d. Did not request adequate assistance, on establishing Patient A may 
have suffered a long fall;  
e. Did not administer Oxygen in a timely manner;  
f. Did not provide a timely update to the Emergency Operations Centre 
requesting assistance;  
g. Did not seek advanced clinical assistance on identification of a 
significant injury.  
2. On or about 29 April 2021, in relation to Service User B (“Patient B”), you: 
a. Did not conduct an adequate examination of Patient B;  
b. Did not administer blood products in a timely manner;  
c. Did not manage the bleeding adequately;  
d. Did not wait the arrival of the air ambulance. 
3. On or about 11 July 2022, in relation to Service User C (“Patient C”), you:  
a. Did not adequately explore with Patient C’s family the potential that 
Patient C had experienced a fall;  
b. Did not obtain an adequate history;  
c. Did not recognise potential ‘red flags’ including that Patient C:  
i. was only communicating using ‘grunting’ noises;  
ii. was not seen to mobilise (walk);  
d. Did not adequately undertake, or make an adequate clinical record of 
undertaking, one or more of the following for Patient C:  
i. A clinical assessment;  
ii. A secondary survey;  
iii. A medical history;  
iv. A Face, Arms, Speech, Time (FAST) test;  
v. A neurological assessment;  
vi. An electrocardiogram (ECG);  
vii. An exploration of his cognitive baseline;  
viii. A standing Blood Pressure.  
e. Did not make an adequate record of the patient’s antiplatelet 
medication;  
f. Did not undertake an adequate handover, or make an adequate 
record of handing over, the care to Patient C’s GP;  
g. Did not make any, or any adequate, record of worsening care advice; 
h. Did not clarify whether Patient C had a birthmark on his head with the 
patient’s family and/or an injury;  
i. Did not make an adequate record of your further assessment on 
Patient C.  
4. The matters set out in particulars 1 to 3 above constitute misconduct and/or 
lack of competence.
