Order

The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 12 months, you, Dr Hannah Puddephatt, must comply with the following conditions of practice:

1. You must confine your professional practice to the treatment of low risk adult patients;

2. You must maintain a record of every case you have undertaken and you must:

a. provide a copy of these records to the HCPC on a 3-monthly basis, the first report to be provided within 3 months of the Operative Date; and

b. make those records available for inspection at all reasonable times by any person authorised to act on behalf of the HCPC.

3. You must place yourself and remain under the indirect supervision of two workplace supervisors, both registered as a Clinical Psychologist by the HCPC, and supply details of your supervisors to the HCPC within 2 weeks of the Operative Date. You must meet with at least one of these workplace supervisors monthly and follow their advice and recommendations. Such supervision may be carried out online.

4. Your workplace supervisors shall provide a joint report to the HCPC every 3 months which includes a summary of your workplace supervision sessions and the profiles of the service users you have treated.  

5. You must inform the HCPC within seven days if you intend to take up any other or further professional work. 

6. You must inform the HCPC within seven days if you take up work requiring registration with a professional body outside the United Kingdom.

7. You must inform the HCPC within seven days of becoming aware of: 

a.  any patient safety incident you are involved in;

b.  any investigation started against you; and

c.  any disciplinary proceedings taken against you.

8. You must inform the following parties that your registration is subject to these conditions: 

a. any organisation or person employing or contracting with you to undertake professional work; 

b. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application); 

c. any prospective employer for professional work (at the time of your application); 

d. any organisation through which you are undertaking professional training.

9. You must allow the HCPC to share, as necessary, details about your performance, compliance with, and/or progress under these conditions with: 

a. any organisation or person employing or contracting with you to undertake professional work; 

b. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);  

c. any prospective employer for professional work (at the time of your application); 

d.  any organisation through which you are undertaking professional training.

Notes

This order will be reviewed by the Committee no later than 15 October 2026 or earlier if new evidence which is relevant to the Order becomes available after it was made.
