As a registered Radiographer (RA41518), whilst employed as a locum Radiographer between 20 April 2023 and 11 August 2023: 

1. You failed to produce scan images of the required quality for up to 278 patients, as listed in Schedule 1. 

2. You failed to optimise equipment settings before undertaking ultrasound examinations on up to 79 patients, as listed in Schedule 2. 

3. You failed to produce accurate and/or clear reports in respect of ultrasound examinations for up to 268 patients, as listed in Schedule 3. 

4. You failed to address clinical questions and/or extend the examination to find a cause of symptoms for up to 27 patients, as listed in Schedule 4.

5. You failed to follow correct procedures for the management of soft tissue lesions for up to 30 patients, as listed in Schedule 5. 

6. You failed to verify up to 18 reports leading to delay in patient care. 

7. The matters set out in particulars 1 – 6 above constitute a Lack of Competence and/or Misconduct. 

8. By reason of your misconduct and/or lack of competence, your fitness to practise is impaired
