That you, during the course of your employment as an Operating Department Practitioner at University Hospital Coventry and Warwickshire NHS Trust:
1. On an unknown date between 10 June 2013 and 6 January 2014, you stole the following items from University Hospital Coventry and Warwickshire NHS Trust premises:
a. 1 bottle of Ketalar, Batch number 70458C
b. 1 cannula
2. Your actions as set out at paragraph 1 above were dishonest.
3. The matters set out above constitute misconduct.
4. By reason of your misconduct your fitness to practise is impaired.
