As a registered Radiographer (RA46204):

1. Between 20 January 2020 and 16 October 2021, you did not provide adequate treatment to service users, in that:

a. On 10 May 2021, you did not carry out adequate safety screening before Service User C’s MRI scan.

b. On 10 May 2021, you did not ask the Cardiac Physiologist to turn off Service User C’s pacemaker.

c. On 16 October 2021, you did not carry out adequate safety screening before Service User A’s MRI scan.

2. On 16 October 2021, you did not report the incident outlined at particular 1d to a senior member of staff and/or submit a Datix report.

3. On 19 October 2021 and/or 20 October 2021, you were not honest about the location it was discovered that Service User A had a pacemaker.

4. On an occasion between 16 October 2021 and 19 October 2021, you asked Colleague A to lie in their witness statement about the location it was discovered that Service User A had a pacemaker.

5. Your conduct in relation to particular 3 was dishonest in that you knew your version of events was not truthful.

6. Your conduct in relation to particular 4 was dishonest in that you knew asking Colleague A to lie would withhold the truth and/or support your version of events.

9. The matters set out in particular 1 above constitute lack of competence.

10. By reason of the matter set out above, your fitness to practise is impaired by reason of your misconduct and/or lack of competence and/or health.
