1. Between January 2022 and August 2022, you:

a. On one or more occasions, made a claim for overtime payment in which the hours claimed were in excess of the hours worked;

b. Claimed payment for shifts on 09 February 2022, 20 February 2022, and/or 01 May 2022, when you had not logged on to the booking system;

c. Claimed an overtime payment for a shift on 01 August 2022 where you were not on shift;

d. On one or more occasions, claimed payment for one or more shifts when you had logged on later than the scheduled start time.

2. On or around 06 September 2022 and/or thereafter, you failed to notify the Health and Care Professions Council (HCPC) that you had restrictions placed on your practice, in that you were not permitted to
undertake any overtime shifts.

3. Your conduct in relation to Particular 1 was dishonest in that you
submitted and/or claimed false payments for hours that you knew you
had not worked.

4. Your conduct in relation to Particular 2 was dishonest in that you did not 
inform the HCPC of restrictions on your practice when you knew that 
you were required to do so. 

5. The matters set out in Particular 1, 2, 3, and/or 4 above constitute 
misconduct. 

6. By reason of the matters set out above, your fitness to practise is 
impaired by reason of misconduct.
