Allegation

As a registered Biomedical Scientist (BS):  


1. On 26 January 2023, in relation to Patient F, you did not document the request from St George’s hospital for cell products in the lab diary  


2. On 26 January 2023, in relation to Patient G, you did not document the request from St George’s hospital for cell products in the lab diary

 
3. On 21 June 2023, in relation to Patient A, you:  


a. did not write the bag or location information on the “Request for Issue of Cryopreserved Products” form  


b. signed and returned a request for a cryopreserved product when you did not carry out the physical checks of the cell product to ensure availability and/or fitness for issue  


c. placed the “Request for Issue of Cryopreserved Products” form in the wrong folder  


4. On 27 June 2023, in relation to Patient A:  


a. you retrospectively wrote a bag number on the request form for the cell product requested  


b. you denied that you had retrospectively written a bag number on the request form for the cell product when you had

 
c. your actions at Particular 4a and 4b were dishonest in that you entered a bag number when you knew this was not the correct bag number and denied doing so  


5. On 23 June 2023, in relation to Patient B, you signed and returned a request for a cryopreserved product when you did not carry out the physical checks of the cell product to ensure availability and/or fitness for issue  


6. On 23 June 2023, in relation to Patient C, you:  


a. did not document the request from St George’s hospital for cell products in the lab diary  


b. placed the patient folder in the wrong part of the visual management tray system  


7. On 07 September 2023, in relation to Patient D, you:

 
a. tested a cell sample for CD34 counts and selected the incorrect sample on the automatic results transfer system (“Artcalcs”) which resulted in calculations having a dilution factor of 1 rather than 10  


b. sent a copy of the interim report of donation to the hospital, reporting incorrect results.  


8. On 03 July 2024, in relation to Patient E, you:  


a. incorrectly tested a harvest sample instead of a predictor sample  


b. did not check the sample against the paperwork before testing it  


c. manually entered the data and / or authorised the data on Artcalcs and did not ask another Biomedical Scientist to sign the paperwork to confirm that the  


9. On 23 July 2024:  


a. you informed RW that you had asked Colleague A to verify the manual entry you made on 03 July 2024 when you had not  


b. your actions at Particular 9a were dishonest in that you knew you had not asked Colleague A to verify your manual entry.  


10. The matters set out in particulars 1 – 9 above constitutes misconduct and/or lack of competence.  


11. By reason of your misconduct and/or lack of competence, your fitness to practise is impaired.
