1. On or around 17 January 2023, you:  
a. Did not and/or were unable to lead on the transfer assessment of Service User A; and:  

i. You did not recognise that Service User A was too fatigued for the transfer assessment to be completed; and/or  
ii. You did not stop the transfer assessment when it was evident 
that Service User A was too fatigued to be assessed.  

2. On or around 19 January 2023, you:  
a. Attempted to conduct a sit-to-stand transfer alone on Service User A, which was not in line with the care plan; 
b. Did not recognise a deterioration in Service User A’s health; and/or  
c. Did not adequately escalate Service User A’s deterioration. 
 
3. Between 4 August 2022 and 20 January 2023, you failed to achieve a satisfactory standard of practice in relation to the following areas:  
a. Time management.  
b. Caseload management.  
c. Clinical reasoning.  
d. Communication skills.  
e. Decision making.  
f. Record keeping.  
g. Working autonomously.
  
4. The matters set out in particulars set out in 2a above constitute misconduct 
and/or a lack of competence.  

5. The matters set out in particulars 1, 2b, 2c and/or 3 above constitute a lack 
of competence.  

6. By reason of the matters set out above, your fitness to practise is impaired 
by reason of misconduct and/or a lack of competence.
