Order

Order: The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 18 months, you, Jamie Miller, must comply with the following conditions of practice:

(1) You must promptly inform the HCPC if you cease to be employed by your current employer or take up any other or further employment.

(2) You must promptly inform the HCPC of any disciplinary or capability proceedings taken against you by your employer.

(3) You must inform the following parties that your registration is subject to these conditions:

A. any organisation or person employing or contracting with you to undertake professional work;

B. any agency you are registered with or apply to be registered with (at the time of application); and,

C. any prospective employer (at the time of your application).

(4) You must place yourself and remain under the supervision of a workplace supervisor who is a Physiotherapist working at Band 6 (or equivalent) or above, and you must supply details of your supervisor to the HCPC within 4 weeks of today’s date.   You must attend upon that supervisor as required and follow their advice and recommendations.

(5) You must provide to the HCPC no later than 2 weeks before the date of any review of this Order an up-to-date report prepared and signed by your supervisor.  The report should comment on your performance generally, and specifically in relation to the following areas: (i) clinical reasoning; (ii) assessment skills; (iii) case load prioritisation and, (iv) communication. 

(6) You must not undertake any out-of-hours or on-call duties.

(7) You must inform your present and any future employer and, if available, the Occupational Health service associated with the employment, of your health condition.

Notes

This Order will be reviewed by the Committee no later than 1 December 2017 or earlier if new evidence which is relevant to the Order becomes available after it was made.
