Allegation

During the course of your employment as a Paramedic for Yorkshire Ambulance Service NHS Trust:

 

1. On 28 April 2015, you did not undertake secondary observations on Patient A;

 

2. On 28 April 2015, you did not:

a) Undertake any further ECG’s;

b) Instruct your crew mate to undertake any further ECG’s;

 

3. On 28 April 2015, in respect of Patient A, you drove the ambulance to the hospital and therefore did not attend to Patient A during conveyance;

 

4. On 28 April 2015, you did not put in place an adequate treatment plan for Patient A;

 

5. On 28 April 2015, you did not check and/or sign Patient A’s Patient Report Form;

 

6. On 5 February 2015, you did not take Patient B directly to the Primary Percutaneous Coronary Intervention (‘PPCI’) service;

 

7. On 5 February 2015, you did not record the following on Patient B’s Patient Report Form:

a) Your arrival time on the scene;

b) SP02;

c) Administration of oxygen;

d) Patient B’s presenting complaint;

e) Time of handover at the hospital;

f) Who completed the observations of Patient B;

g) Patient B’s Glasgow Coma Score (‘GCS’);

h) Patient B’s pain score;

i) The correct date of the incident;

 

8. On 5 February 2015, noting that Patient B was bradycardic, you:

a) administered Naloxone; and/or

b) did not administer Atropine;

 

9. On 5 February 2015, you did not pre-alert the hospital in relation to Patient B in a timely manner or at all;

 

10. On 5 February 2015, you did not complete a 12 lead ECG on Patient B;

 

11. Between August 2015 and 7 April 2016, you worked with an ECA on up to 11 occasions;

 

12. Whilst working as part of a double-ECA crew you discharged the following patients on scene:

a) Patient C on 27 November 2015;

b) Patient D on 17 November 2015;

c) Patient E on 26 March 2016;

 

13. Your actions described in particulars 1 to 12 constitute misconduct and/or lack of competence;

 

14. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
