Allegation

While working as a registered Social Worker for Hampshire County Council:

1. Regarding Service User 1, :

(a) in or around April 2014, Service User 1’s wife told you she was considering moving Service User 1’s capital (the Conversation) and you did not make a contemporaneous record of the Conversation

(b) You did not take and/or record any action in response to the Conversation

(c) You did not follow the 8 May 2014 instructions from Team Manager A to:

i. Make a record of the Conversation

ii. Inform Service User 1’s wife that this would be seen as a deliberate deprivation of capital until 9 June 2014

2. Regarding Service User 2:

(a) Between October 2013 and January 2014:

i. You did not recognise and/or record that Service User 2’s capital had dropped

ii. You did not investigate and/or record investigating whether Service User 2’s money was being managed appropriately

iii. You did not contact and/or record contacting the Financial Assessments and Benefits team (FAB) to report the drop in capital

(b) Between January 2014 and May 2014:

i. You did not record contacting FAB

ii. You did not chase FAB for a response

iii. You did not ask for evidence as to why Service User 2’s capital had dropped

(c) Between May 2014 and July 2014:

i. You did not record the review completed on 20 May 2014

ii.You did not update Service User 2’s home address on file

3. Regarding Service User 3, between January 2014 and May 2014:

(a) You did not update Service User 3’s record to reflect a change in the care start date

(b) You did not refer Service User 3 to FAB for a financial assessment until 9 May 2014

(c) You did not inform and/or record informing Service User 3 of the charging policy and/or discuss financial contributions until 9 May 2014

4. Regarding Service User 4, you did not ensure that Take a Break care was put in place and/or delayed the care start date from 29 January 2014 until 23 April 2014

5. Regarding Service User 5:

(a) You did not request a care package between 29 January 2014 and 27 February 2014

(b) You delayed the care start date from 26 March 2014 until 9 or 10 April 2014

(c) Your 21 March 2014 note incorrectly stated the new start date was 10 March 2014 when it should have said 9 or 10 April 2014

(d) You did not contact and/or record contacting the community matron between 22 January 2014 and 9 May 2014

(e) You did not refer and/or record referring Service User 5 to a befriending service as agreed at the 24 June 2014 review

(f) You did not complete and/or record follow up on Service User 5’s concerns about care agency staff raised on or around 5 September 2014

(g) You did not contact and/or record contacting the community matron following your call with Service User 5 on 15 September 2014

(h) You did not complete the record of the 4 November 2014 review until around 5 December 2014

(i) You fell asleep during meeting with Service User 5 in or around November 2014

(j) You did not follow Team Manager A’s instruction to email Team Manager B when the 4 November 2014 review was sent for authorisation

(k) You did not ensure that the review due date was accurately recorded

6. Regarding Service User 6:

(a) You submitted pre-panel applications on the following dates without providing evidence as to why Service User 6 cannot be left alone:

i. 11 February 2014

ii.4 March 2014

iii. 13 May 2014

iv. 17 June 2014

(b) You did not record that the 4 March 2014 application had been rejected

(c) You made the applications listed at particulars 6ai-iv, which were not necessary because Service User 6 did not require that level of care

7. Regarding Service User 7:

(a) You included inappropriate and / or irrelevant information in the assessment completed in or around April 2014

(b) You did not ensure that the provision was updated correctly between 18 March 2014 and 21 July 2014

8. Regarding Service User 8:

(a) On or around 1 April 2014 you inaccurately recorded that Service User 8 required three care calls daily

(b) You did not tell and/or record telling Service User 8 that lunch calls would not be provided until 3 April 2014

9. Regarding Service User 10, you did not adequately record and/or complete the carer’s assessment between 4 June 2014 and 21 July 2014.

10. Regarding Service User 11, you did not adequately record and/or complete the assessment between 4 June 2014 and 21 July 2014

11. Regarding Service User 12:

(a) You recommended that live-in care be continued on or around 4 June 2014 without adequate justification for doing so

(b) You did not ensure that appropriate care was in place when the care package was due to end on or around 8 June 2014

12. Regarding Service User 13:

(a) You did not send the assessment completed on 9 June 2014 for authorisation until on or after 21 July 2014

(b) You did not contact and/or record contacting Service User 13 between around 4 July 2014 until around 21 July 2014

(c) You did take and/or record any other action to progress the case between around 4 July 2014 until around 21 July 2014

(d) You told Service User 13’s son that they would need to pay for telecare equipment privately, which was not the case

(e) You did not contact and/or record contacting Service User 13’s son to inform him that adult services would fund telecare between 19 November 2014 and 16 February 2015

(f) You did not use the correct form when arranging for care to increase between 26 November 2014 and 10 February 2015

13. Regarding Service User 14:

(a) You did not contact and/or record contacting Service User 14’s daughter after 19 September 2014

(b) You did not record the safeguarding concern in the Adults Intergraded System (AIS) safeguarding module between 6 November 2014 and 1 December 2014

(c) You did not check that Service User 14’s contribution was being paid between 6 November 2014 and 1 December 2014

(d) You did not complete and/or record a visit to Service User 14 on 25 November 2014

(e) You did not attempt to and/or record attempting to contact Service User 14’s daughter between 4 December 2014 and 28 January 2015

14. Regarding Service User 15:

(a) You did not record the 18 August 2014 review until around 28 October 2014

(b) You did not request that the Provision Support Officer (PSO) complete and/or record provision between 18 August 2014 and 21 September 2014

(c) You did not contact and/or record contacting Service User 15’s wife to discuss her concerns between 23 September 2014 and 28 October 2014

15. Regarding Service User 16, after the Free Crisis Care Ended on 27 August 2014:

(a) You did not review the care arrangements

(b) You did not contact and/or record contacting Service User 16 to discuss their care

(c) You did not contact and/or record contacting the care agency to discuss Service User 16’s care

16. Regarding Service User 17, you did not complete the recording of the review between 3 November 2014 and March 2015

17. Regarding Service User 18:

(a) You did not take and/or record any action in response to the safeguarding concerns raised on 23 July 2014 in a timely manner

(b) You did not record who agreed to close the case and/or take no further action

(c) You did not complete the safeguarding module on AIS

18. Regarding Service User 19:

(a) During a conversation on or around 29 August 2014, you incorrectly told Service User 19s son that:

i. Service User’s 19’s move to the care home could not take place and/or would need to be postponed

ii. Funding had not been agreed for Service User 19’s care

iii. Service User 19’s son would need to pay for and/or arrange transporting Service User 19 to the care home

(b) You did not record the conversation listed above at particular 19(a)

19. Regarding Service User 20, you did not provide Team Manager A with a summary of concerns between 13 November 2014 and 9 January 2015

20. Regarding Service User 21:

(a) You did not contact and/or record contacting Service User 21 to update her on the situation until 22 October 2014

(b) You did not ensure that the assessment completed on 29 August 2014 was authorised between 29 August 2014 and 22 October 2014

(c) You did not contact and/or record contacting Service User 21 or Service User 21’s husband to update them on the situation between 22 October 2014 and 28 November 2014

21. Regarding Service User 22:

(a) You did not review Service User 22’s bank account and/or Direct Payment paperwork between 4 September 2014 and 4 March 2015

(b) You did not visit and/or record visiting Service User 22 between 6 November 2014 and 4 March 2015

(c) You did not attempt and/or record attempting to contact Service User 22 between 20 November 2014 and 4 March 2015

22. Regarding Service User 23:

(a) You did not identify and/or record whether Service User 23 wanted a referral to the police between 6 November 2014 and 4 March 2015

(b) You did not record the completed review between 6 November 2014 and 1 December 2014

(c) You did not send the completed review for authorisation between 6 November 2014 and 4 March 2015

23. Regarding Service User 24, on or around 29 October 2014 you recommended the case for closure, although you had not:

(a) Sent the assessment for authorisation

(b) Request that the PSO alter the provision

24. Regarding Service User 25:

(a) You did not record the 12 November 2014 visit:

i. On the case file

ii. On the Outlook diary

(b) You did not check with Service User 25 that they were happy with the language used in your assessment in a timely manner

(c) You did not re-arrange and/or record re-arranging the visit to Service User 25 which was due to take place on 21 November 2014

25. Regarding Service User 26: , you did not complete an adequate Mental Capacity Assessment (MCA) between 6 November 2014 and 23 February 2015

26. Regarding Service User 27:

(a) You did not send the 22 October 2014 assessment for authorisation until 10 February 2015

(b) You did not contact and/or record contacting Service User 27 and/or their family between around November 2014 and 4 March 2015

(c) You did not complete a PSO request for provision until 4 March 2015

(d) You did not complete a FAB referral between 21 October 2014 and 4 March 2015

27. Regarding Service User 28:

(a) You did not complete and/or record an MCA between 30 October 2014 and 12 December 2014, despite instructions from Team Manager A

(b) You advised the care home to make a Depravation of Liberties application in or around November 2014, despite there being no MCA in place

28. Regarding Service User 29, you did not reassess and/or update the level of care required after Service User 29 had several admissions to hospital in or around December 2014

29. Regarding Service User 30, between 12 November 2014 and 9 January 2015:

(a) You did not complete and/or record an MCA

(b) You did not complete and/or record a Best Interest decision

30. Regarding Service User 31, :

(a) You made an FAB referral on 19 November 2014 and:

i. You did not include the care needs assessment date

ii. You had not recorded the assessment

(b) You did not arrange for a nursing home placement between 19 November 2014 and 2 February 2015

(c) You advised Team Manager A on 9 January 2015 that arrangements had been made for a nursing home placement, when this was not the case

31. Regarding Service User 32, following allocation to you on 14 November 2014:

(a) You did not complete and/or record any safeguarding investigation

(b) You did not inform Team Manager A of any safeguarding investigation you had undertaken

32. The matters set out in paragraphs 1-32 constitute misconduct and/or lack of competence

33. By reason of your misconduct and/or lack of competence your fitness to practise is impaired
