During the course of your employment as a Social Worker by Gloucestershire County Council, you:

1. In the case of Child A:

A. In or around July 2014, did not ensure a risk assessment was undertaken before Child A visited her mother;

B. In or around September 2014, on being made aware of serious allegations made by Child A, you:

i) did not ensure that a strategy discussion and/or meeting was held;

ii) did not follow the ‘need to know procedure;

iii) did not follow the ‘allegation management procedure;

D. Did not maintain adequate records in relation to the case, in that you:

i) did not ensure that reflective discussions with the duty social worker were recorded,

ii) did not ensure that professional challenges to the social worker were recorded,

iii) did not record any management decision,

2. In the case of Child B, Between June 2014 and August 2014, on being made aware of a disclosures by Child B that his/her foster carers had hurt him/her, you:

i) did not ensure that a strategy discussion and/or meeting were held to discuss the allegations.

3. In the case of Child C:

A. In or around October 2014, following serious allegations which relate to Child C, you did not:

i) take any action under the relevant child protection procedures to notify your manager of the allegation;

ii) ensure that a strategy discussion and/or meeting took place in a timely manner;

iii) ensure that discussion with Gloucestershire Police were recorded;

iv) ensure that the appropriate checks were undertaken to establish if Service User A was known to the local authority.

4. In the case of Children E:

A. On 08th September 2014 and/or 20th November 2014, delayed in responding to a complaint, which caused the service users to miss their farewell contact with their siblings.

B. did not write an age appropriate response to the two children aged 8 and 10

5. The matters described in paragraphs 1 4 constitute misconduct and/or lack of competence.

6. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
