 Between approximately 19 February 2015 and 6 July 2015, during the course of your employment as a Social Worker for Wiltshire County Council:
1. You did not record whether you had spoken to an individual who was alleged to have sent indecent images to Service User A

2. In respect of Service Users B and C:
a) You did not record whether you had visited Service User C, following a referral which disclosed that Service User C was assaulted by her partner when Service User B was present in the home.
b) You did not record any evidence of the progression of this case

3. In respect of Service User D:
a) You recorded 6 core group meetings minutes from February 2015 to April 2015 that did not take place.
b) Your core group meeting record for April 2015 was copied from February 2015’s core group meeting record
c) You did not review the child protection plan which was derived from core group meetings which took place from January 2015 to April 2015.
d) You did not record any information in respect of the service user’s father and his involvement with Service User D in your Core group meeting records

4. In respect of Service User E:
a) You did not provide Service User E’s mother with documents pertaining to the child protection plan that was in place
b) You did not record any visits to Service User E and/or Service User E’s mother between February 2015 and April 2015.

5. In respect of Service Users F, G and H:
a) Your RCPC report dated 15 May 2015:
i) did not provide a clear understanding of current risk to Service User F and siblings
ii) did not provide sufficient evidence to support your recommendation to end child protection planning in favour of child in need planning
iii) recorded a contradictory comment in that you recorded that the risk to the children was reduced due to the father leaving the family home, but then make reference to not knowing whether the parents continued in a relationship
b) You advised Service User G not say anything about her partner living with her during a child protection conference
c) You recorded a visit on the 23 March 2015 which described Service User F and H’s bedroom without seeing it

6. In respect of Service User I:
a) You stated within a Single assessment report that a request for police checks were sent to the police on the 19 February 20165, when this did not happen
b) Your any records for visits recorded between approximately February 2015 to July 2015 did not contain sufficient detail.

7. In respect of Service User J between February 2015 and May 2015:
a) You did not complete and/or record a Single assessment
b) You did not record any contact with the children and/or parents
c) You did not record any contact you made with any supporting agencies

8. In respect of Service Users K, L and M:
a) Your Section 47 report:
i) stated that you undertook a Section 47 visit with the police, when this did not happen.
ii) Did not record whether any consideration had been given for a medical examination
iii) did not consider Service user L’s relationship with his father and the effects on Service User L
iv) did not adequately assess past harm or current risks to Service Users K and L
b) You did not record any visits in respect of Service Users K, L and M other than one attempted visit on 1 April 2015;
c) You did not record whether you had spoken to Service User K

9. Your actions as described at 5c) and 6a) were dishonest.

10. Your actions as described at 3a), 5c), 6a), and 8a) i) and ii) constitute misconduct.

11. The matters as described in paragraphs 1 - 9 constitute misconduct and/or lack of competence.

12. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
