Allegation

During your employment as a Social Worker for Hertfordshire County Council, from approximately 31 March 2014 to 02 June 2015;

1. In relation to Service Users D and E, you:

a) did not complete an adequate Child & Family assessment in approximately May/June 2014 in that the assessment did not include:

i. your observations of the children’s emotional and behavioural development;

ii. an adequate analysis of the risk;

b) did not complete the assessment referred to in paragraph 1a) in a timely manner;

2. In relation to Servicer User F, on or around 17 September 2014 you;

a) did not adequately conduct a home visit in that you did not assess and/or record:

i. information of the risk factors;

ii. information about historic concerns;

b) undertook a visit to the child while you were unaccompanied;

c) did not complete an adequate record of a home visit to the mother that took place on or around 22 September 2014, in that you did not include:

i. that empty beer bottles were visible in the garden;

ii. relevant information provided by the mother;

3. In relation to Service Users G, you:

a) On or around 30 June 2014, breached confidentiality in that you gave the mother a report relating to a different family;

b) Did not immediately report the data breach referred to in paragraph 3a) to a senior person;

c) On or around 30 June 2014:

i. Did not explain the contents of the Child Protection Conference report to the mother;

ii. Did not explain the importance of a case conference to the mother;

d) Did not complete an adequate Child Protection Conference report in that it had to be revised on more than one occasion;

e) On or around 3 July 2014 shared the report referred to at paragraph 3c) at a Child Protection Conference without making the changes required;

4. On or around 24 June 2014, while observing a Child Protection Conference, you expressed a view that was different to that of the allocated Social Worker.

5. The matters as described in paragraphs 1 – 4 constitute misconduct and/or lack of competence.

6. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
