The Interim Conditions of Practice Order originally imposed on 16 September 2016, varied on 6 March 2017 and maintained on 2 June 2016 and 31 August 2017 is confirmed as set out below:

1. You must promptly inform the HCPC if you cease to work at Physiotherapy Clinic A, Birmingham.

2. You must promptly inform the HCPC of any disciplinary proceedings taken against you.

3. You must inform the following parties that your registration is subject to these conditions:

A. any organisation or person employing or contracting with you to undertake professional work;

B. any agency you are registered with or apply to be registered with (at the time of application); and

C. any prospective employer (at the time of your application).

4. You must confine your professional practice at all times to Physiotherapy Clinic A, Birmingham.

5. You must not carry out any Podiatric assessment or treatment unless directly supervised at all times by a registered Physiotherapist or Podiatrist, neither of whom must be employed by you directly or indirectly.

6. You must inform your supervisor of the details of the allegations against you.

7. You must maintain a record of every case where you have undertaken Podiatric assessment or treatment which must be signed by your supervisor and you must:

A. provide a copy of these records to the HCPC on a monthly basis, the first report to be provided by 16 October 2016, or confirm that there have been no such cases during that period; and

B. make those records available for inspection at all reasonable times by any person authorised to act on behalf of the HCPC;

C. provide evidence to confirm that each patient has given informed consent to their examination.

8. You must not undertake any out-of-hours work or on-call duties.


This order will be reviewed by the Committee no later than 23 February 2018 or earlier if new evidence which is relevant to the Order becomes available after it was made.
