1. Between 30 July 2012 and 24 February 2015, during the course of your employment as a Social Worker by Blackburn and Darwen Borough Council, you:
a) In relation to Service User A:
i. Did not challenge Person A’s breach of bail conditions and/or;
ii. Did not inform your Area Team Manager and/or the Police of the breach:
a) immediately; and/or
b) in a timely manner
b) In relation to Service User B, who had allegedly been assaulted in approximately August 2014:
i. Did not take action and/or intervene following the allegation of assault in a timely manner;
ii. Did not report the allegation to your Area Team Manager in a timely manner;
iii. Did not speak to Service User B alone and/or only sought the views of the adults and/or parents involved;
iv. Did not offer support to Service User B’s mother, despite there being a history of domestic violence
c) In relation to Case C:
i. Did not undertake an assessment and/or arrange for an assessment to be undertaken of Service User C’s younger brother;
ii. Misled your Area Team Manager and/or other professionals by informing them that you were undertaking an assessment of Service User C’s younger brother following concerns made about his sibling.

2. Between 22 February 2016 and 4 March 2016, during the course of your employment as
a Social Worker at Cumbria County Council, you:
a) On or around 01 March 2016, whilst on a visit to Child A’s family home:
i. Suggested to Child A’s mother and/or Person B that it was wrong to remove shotguns from the family home, or words to that effect;
ii. Advised Child A’s Mother and/or Person B that Child A should be encouraged to shoot guns on the family farm so he had an interest, or words to that effect.

3. Your actions described in paragraph 1(c)(ii) were dishonest.

4. The matter described in paragraph 1(c)(ii) was misconduct.

5. Your actions described in paragraphs 1 and/or 2 constitute misconduct and/or lack of competence.

6. By reason of your misconduct and/or lack of competence, your fitness to practise is impaired.
