The Interim Conditions of Practice Order made on 1 November 2017 is varied. The conditions are as follows:

You, Ms Mary Squires, must comply with the following interim Conditions of Practice:

1. You    must    remain    under    the    care    of   a   psychiatrist and your General Practitioner (GP) and/or occupational health practitioner until such time as they discharge you from their care. 

2. You must inform your psychiatrist and general practitioner about these interim conditions of practice and authorise each of them to provide the HCPC with any relevant information about your health and any treatment you are receiving.

3. An updated report from a health professional involved in your care must be submitted by you to the HCPC at least 14 days before each review hearing.

4. You must not return to employment as a Social Worker without the written agreement of your psychiatrist, GP and/ or occupational health practitioner.

5. When you return to work as a Social Worker, you must receive regular clinical supervision from a Social Worker registered with the HCPC in addition to any management supervision you may receive. 

6. When you return to work as a Social Worker, you must authorise your clinical supervisor to provide reports on your progress to reviewing panels who review your case during the term of this order. Such reports shall be submitted by you to the HCPC at least 14 working days prior to a review hearing.

7. You  must  keep  your  professional  commitments  under  review  and  limit  your professional practice in accordance with the advice of your psychiatrist and/or general practitioner.

8. You must cease practising immediately if you are advised to do so by your psychiatrist and/or GP.

9. You must promptly inform the HCPC if you cease to be employed by your current employer or take up any other or further employment.

10. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

11. You must inform the following parties that your registration is subject to these conditions:

a. any organisation or person employing or contracting with you to undertake professional work;

b. any agency you are registered with or apply to be registered with (at the time of application); and

c. any prospective employer (at the time of your application).

Notes

This order will be reviewed by the Committee no later than 22 April 2018 or earlier if new evidence which is relevant to the Order becomes available after it was made.
