Order
The Registrar is directed to annotate the Register, on an interim basis to show that, for a period of 18 months from today’s date, you, Ms Munirah Mohamed Sulaiman (DT032098), must comply with the following conditions of practice:
1) You must confine your professional practice to a single employer, excluding agency and bank work.
2) You must place yourself and remain under the supervision of a clinical supervisor (at least a band 6 Dietitian), registered by the HCPC or other appropriate statutory regulator and supply details of your supervisor to the HCPC within 7 days of starting with an employer.
3) You must attend upon that supervisor as required and follow their advice and recommendations. That supervision must include individual supervision and may include, if advised by the supervisor, peer group discussion.
4) You must maintain a record of every case where you have undertaken clinical work as a dietitian, which must be reviewed by a supervisor as soon as practicable after the work being undertaken.
5) You must make these case records available to inspection at all reasonable times by any person authorised to act on behalf of the HCPC.
6) You must allow your supervisor to provide information to the HCPC about your progress and performance as a Dietitian.
7) You must promptly inform the HCPC if you cease to be employed by your current employer or take up any other or further employment.
8) You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.
9) You must inform the following parties that your registration is subject to these conditions:
a) any organisation or person employing or contracting with you to undertake professional work;
b) any prospective employer (at the time of your application).
Notes
This order will be reviewed by the Committee no later than 18 July 2023 or earlier if new evidence which is relevant to the Order becomes available after it was made.
Right to Apply to the Court
Under Article 31(12) of the Health Professions Order 2001, you may apply to the appropriate court to revoke or vary the conditions imposed
