b. Failed to report the subsequent loss of the hospital ID card.

4. The matters set out at paragraphs 1-2 were dishonest.

5. The matters set out at paragraphs 1-4 amount to misconduct.

6. By reason of your misconduct, your fitness to practise is impaired.
