Ms Pinkie Uddin

As a registered Speech and Language Therapist (SL33971) your fitness to practise is impaired by reason of misconduct and/or lack of competency. In that

1. During 2016 and 2017, while employed at the Whittington Health NHS Trust, you:

a) Did not complete and/or record in Rio, on the dates specified in Schedule A:

i. any contacts with child service users and/ or

ii. administrative tasks

b) Did not provide the required number of speech and language therapy sessions as outlined in each child’s Education, Health and Care Plan, for the child service users outlined in Schedule B

c) Did not visit schools 1, 2, 3 and/or 4 for the required number of days in that you:

i. Visited School 1 on 22 out of 47 days

ii. Visited School 2 on 16 out of 24 days

iii. Visited School 3 on 9 out of 16 days

iv. Visited School 4 on 11 out of 24 days

d) Provided untrue and/or unverified reasons for rescheduling and/or cancelling visits at schools 1, 2 and 4, as outlined in Schedule C.

e) Did not complete clinical notes to an appropriate standard, in that they lacked detail and/or were not contemporaneous.

2. On or around 20 July 2017, while employed at the Whittington Health NHS Trust, you provided an untrue reason for your unauthorised absence from the Islington Council Offices.

3. On or around 14 August 2017, while employed at the Whittington Health NHS Trust, you provided a statement to your employer which contained falsified clinical notes as outlined in schedule D.

4. Between January 2018 to July 2018, while employed at the London North West University Healthcare Trust, you:

a) Provided untrue and/or conflicting reasons for rescheduling and/or cancelling visits at schools 5, 6, 8 and/or 9, as outlined in Schedule E.

b) Did not complete and/or record in System 1, on the dates specified in Schedule F,

i. Contacts with child service users

ii. Contemporaneous case notes in relation to contacts with child service users

5. On or around 21 March 2018, while employed at the London North West University Healthcare Trust, you did not complete case notes following planning meetings for child service users;

a) Child Service User 54

b) Child Service User 55

c) Child Service User 56

d) Child Service User 57

6. Your actions in paragraphs 1(d), 2, 3 and 4(a) were dishonest

7. The matters set out in paragraphs 1-6 amount to misconduct and/or lack of competence

8. By reason of misconduct and/or lack of competence your fitness to practise is impaired
