Allegation
Whilst registered as a Speech and language therapist,
1) In relation to Family A,
A. You did not provide Family A with a Speech and Language Assessment report they paid for, in a timely manner.
B. Between 09 February 2017 and 28 August 2018, you did not respond to Family A’s written correspondences, in respects of the Speech and Language Assessment report they paid for
2) In relation to Family B,
A. You did not provide Family B with a Speech and Language Assessment report they paid for, in a timely manner.
B. Between 31 March 2017 and 07 June 2017, you did not respond to Family B’s written correspondences in respects of the Speech and Language Assessment report they paid for.
3) In relation to Family C,
A. You did not provide Family C with a Speech and Language Assessment report, in a timely manner
B. Between 21 April 2017 and 04 October 2017, you did not respond to Family C’s written correspondences in respects of the Speech and Language Assessment report
C. You did not return a copy of Person A’s medical record in a timely manner.
4) Your actions at paragraphs 1 – 3 constitute misconduct
5) By reason of your misconduct, your fitness to practise is impaired
