Allegation
The allegation against you is now as follows:
As a registered Physiotherapist (PH32296) your fitness to practise is impaired by reason of misconduct and/or lack of competence, in that:
1. During the course of your employment at Betsi Cadwaladr University Health Board between 2007 and 2017, you failed to maintain adequate records for patients, in that you did not
consistently:
- a. Complete and/or upload records of appointments with musculor skeletal physiotherapy patients;
- b. Record key information in your handwritten notes of appointments and/or sign the records including for one or more of the records listed in Schedule A:

2. Did not keep patient records appropriately in that:
- a) you stored up to 591 patient records away from Health Board Premises;
- b) you did not ensure that up to 591 patient records were stored securely;
- c) up to 591 patient records and/or information about their location were not available to other practitioners.
3. The matters set out in paragraphs 1 – 2 constitute misconduct and/or lack of competence.
4. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
