Allegation
1. On 23 March 2018, you took a patient from Laburnum Lodge Intermediate Care unit to their home address and to the bank, and;
a. It was inappropriate to remove the patient from the care unit.
b. You did not complete adequate risk assessments.
2. On 18 April 2018, you instructed a patient’s wife to mobilise the patient when it was inappropriate for them to do so.
3. On 23 April 2018 and 3 May 2018, you displayed aggressive behaviour towards your supervisors during a meeting.
4. On 3 May 2018, you inappropriately assessed a patient, when a Band 6 Supervisor was not present.
5. Between 10 May 2018 & 30 August 2018, you completed private work for Tara Physiotherapy, whilst signed off on sick leave.
6. The matters set out at paragraph 5 were dishonest.
7. Your actions at paragraphs 1 – 6 amount to misconduct.
8. By reason of your misconduct your fitness to practise is impaired.
