Mr Christopher Doe

As a registered Operating Department Practitioner (ODP32915) your fitness to
practice is impaired by reason of misconduct and/or a health condition, in that:

1. Between 06 April 2020 and 18 May 2020 you made unauthorised changes to Spire Healthcare's Controlled Drug Registers, in that you overwrote and/or amended entries made on:

a. 06 January 2020 for the amount of Pethidine that was administered and/or
destroyed and/or the balance that remained in stock in relation to Patient 1;

(i) Your amended entry did not correspond with Witness 4's version of
events and/or Patient 1 's anaesthetic records and/or left 1 Pethidine
50mg/2ml ampoule unaccounted for;

b. 25 March 2020, for the amount of Diamorphine that was supplied and/or the
balance that remained in stock in relation to Patient 2;

(i) Your amended entry did not correspond with Witness 2's version of
events and/or left 1 Diamorphine 5mg/1ml ampoule unaccounted for;

c. 30 April 2020, for the amount of Fentanyl that was supplied and/or the
balance that remained in stock in relation to Patient 3;

(i) Your amended entry did not correspond with Witness 7's version of
events and/or left 1 Fentanyl 100 mcg/2ml ampoule unaccounted for;

d. 30 April 2020, for the amount of Alfentanil that was administered and/or
destroyed and/or the balance that remained in stock in relation to Patient 3;

(i) Your amended entry did not correspond with Patient 3's anaesthetic records
and/or Witness 6 and/or Witness S's version of events and/or left 2 Alfentanil 1mg
12ml ampoules unaccounted for

e. 30 April 2020, for the amount of Morphine that was supplied and/or
administered and/or destroyed and/or the balance that remained in stock in
relation to Patient 3;

(i) Your amended entry did not correspond with Patient 3's anaesthetic
records and/or Witness 6 and/or Witness S's version of events and/or left
2 Morphine 1 0mg/1 ml ampoules unaccounted for

f. 05 May 2020, for the amount of Fentanyl that was supplied and/or
administered and/or destroyed and/or the balance that remained in stock in
relation to Patient 4;

(i) Your amended entry did not correspond with Patient 4's anaesthetic
records and/or left 1 Fentanyl 100 mcg/2ml ampoule unaccounted for

2. Between 06 April 2020 and 18 May 2020 you recorded in Spire Healthcare's
Controlled Drugs Register that Morphine had been supplied and/or
administered and/or destroyed on 05 May 2020 in relation to Patient 4,
when:

(i) this did not correspond with Patient 4's anaesthetic records and/or
Witness 1 's version of events and/or left 1 Morphine 10 mg/lml ampoule
unaccounted for.

3. Between 06 April 2020 and 18 May 2020 you did not undertake an adequate
stock balance transfer from Spire Healthcares Controlled Drugs Register
THRB40 to THRB010520, in that you:

a. overwrote and/or amended the amount of stock being transferred, in respect
of:

i) Alfentanil
ii) Morphine
iii) Fentanyl

b. did not obtain a counter signature when closing down THRB40 to evidence
correct stock transfer

4. Your conduct in relation to particular 1-3 above was dishonest.

5. You have a physical and/or mental health condition as set out in Schedule A

6. The matters described at 1-4 above constitute misconduct.

7. By reason of your misconduct and/or health your fitness to practice is
impaired.
