Mr Steve Weeks

While registered as an Operating Department Practitioner with the Health and Care Professions Council:

1) Between 2016 and 2018, without authorisation from your employer, you removed from your place of work:

a) Diamorphine;

b) Morpine;

c) Syringes;

d) Needles;

2) Between 2016 and 2018, added false entries to your employer’s controlled drug book;

3) On 24 January 2018, you falsified the signature of Colleague 1 when removing 2x 10mg Diamorphine;

4) On 12 May 2018, you falsified the signature of Colleague 3 when removing 1x 10mg Diamorphine;

5) On 12 July 2017, you falsified the signature of Colleague 2 when

removing 1x 10mg Diamorphine;

6) On an unknown date after December 2017, you self-administered Diamorphine and/ or Morphine at your place of work;

7) Your conduct in paragraph 1-5 was dishonest;

8) The matters set out in paragraphs 1-7 constitute misconduct;

9) By reason of your misconduct your fitness to practise is impaired.
