Allegation
The allegation against you is as follows:
As a registered Paramedic your fitness to practise is impaired by reason of misconduct. In that:
1. In or around March 2018, on your application form to North West Ambulance Service you did not declare your previous employment in 2002 with the former Mersey Regional Ambulance Service [ MRAs], and:
a. You did not provide details of the employment within the:
i. PES Clinical Practise Trainer application form
ii. Curriculum Vitae
iii. Practise Education Facilitator application form
iii. Interview
b. You did not declare that you had been dismissed from employment.
2. On 18 May 2017, when you signed and submitted your application form to join the Health and Care Professions Council’s register, you
did not tick the box “ yes” in response to the question: “Have you been disciplined by … your employer?”
3. You did not declare to the HCPC in a timely manner that on or around 29 May 2019 you were dismissed from North West Ambulance Service Trust.
4. Your conduct in relation to allegation 1, 2, and 3 was dishonest.
5. The matters set out in paragraph’s 1 – 4 above constitute misconduct.
6. By reason of your misconduct your fitness to practise is impaired.
