ORDER: The Registrar is directed to annotate the Register on an
interim basis to show that, for a period of eighteen (18) months, you,
Ms Bethany Henry, must comply with the following conditions of
practice:
1.You must restrict your employment as a paramedic to Yorkshire
Ambulance Service NHS Trust (the Trust).

2.You must place yourself and remain under the supervision of a
Workplace Supervisor, registered by the HCPC, and supply details of
your supervisor to the HCPC within two weeks of the operative date.
You must attend upon that Workplace Supervisor as required and
follow their advice and recommendations.

3. You must provide a report to the HCPC from your Workplace
Supervisor at least two weeks in advance of any scheduled review
hearing. The report should include an update in relation to:

• any clinical practice surrounding any patient under the age of
one;
• any Clinical Action Plan in place/completed.

4.You must provide to the HCPC, within seven days, details of any
Clinical Action Plans that the Trust imposes on your practice.

5.You must promptly inform the HCPC if you cease to be employed by
your current employer.

6.You must promptly inform the HCPC of any disciplinary or
competency proceedings taken against you by your employer.

7.You must inform the following parties that your registration is
subject to these conditions:

a. Any organisation or person employing or contacting with you to
undertake professional work;
b. Any prospective employer (at the time of application).

This order will be reviewed by the Committee no later than 21 October 2023 or earlier
if new evidence which is relevant to the Order becomes available after it was made.
