The Registrar is directed to annotate the HCPC Register to show that for a period of 18 months from the date that this Order takes effect (“the Operative Date”), you, Miss Anar Lakha, must comply with the following conditions of practice:

1. You must place yourself and remain under the supervision of a workplace supervisor registered by the HCPC or other statutory regulator. You must supply details of your workplace supervisor to the HCPC within 14 days of the workplace supervisor being identified. You must attend upon that workplace supervisor as required and follow their advice and recommendations.

2. You must not undertake any ultrasound scans unless directly supervised by an HCPC-registered radiographer who is a qualified sonographer, until you have been signed off as competent by your workplace supervisor in the relevant area of ultrasound.

3. You must meet with your workplace supervisor on a weekly basis to consider and reflect on any conduct and/or competence issues in relation to your practice that might arise. After three months, and subject to the recommendation of your workplace supervisor, such meetings can be held less frequently.

4. You must allow your workplace supervisor to provide information to the HCPC about your progress and any concerns relating to your conduct and/or competence no later than 5 days before any review of this Interim Order.

5. You must promptly inform the HCPC if you cease to be employed by your current employer or take up any other or further employment.

6. You must promptly inform the HCPC if you become subject to any further disciplinary proceedings.

7. You must inform the following parties that your registration is subject to these conditions:

A. Any organisation or person employing or contracting with you to undertake professional work;

B. Any agency you are registered with or apply to be registered with (at the time of application); and

C. Any prospective employer (at the time of your application).

This Interim Order will be reviewed by 09 February 2024.
