Order:
The Registrar is directed, on an interim basis, to annotate the Register to show that for a period of 12 months the registration of Ms Lorna James is subject to an Interim Conditions of Practice Order.


The Interim Conditions of Practice are:
1. You must place yourself and remain under the supervision of a workplace supervisor who is registered by the HCPC or other appropriate statutory regulator and supply details of your supervisor to the HCPC within 14 days of taking up employment as a Physiotherapist. You must attend upon that workplace supervisor as required and follow their advice and recommendations.


2. You must work with your workplace supervisor to formulate a Personal Development Plan designed to address the deficiencies in your practice, namely record-keeping.


3. Within three months of taking up employment as a Physiotherapist you must forward a copy of your Personal Development Plan to the HCPC.


4. You must supply your workplace supervisor on a weekly basis with a list detailing all the patients you have treated that week.


5. You must meet with your workplace supervisor on a monthly basis, or more frequently if your workplace supervisor considers it necessary, to consider your progress towards achieving the aims set out in your Personal Development Plan.


6. You must allow your workplace supervisor to provide information to the HCPC about your progress towards achieving the aims set out in your Personal Development Plan.


7. You must inform the HCPC within 14 days if you take up any employment requiring HCPC registration, or if you cease to be employed by an employer who required you to have HCPC registration.


8. You must inform the HCPC within 14 days of any disciplinary proceedings taken against you by your employer.


9. You must inform the following parties that your registration is subject to these conditions:


A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with (at the time of application); and
C. any prospective employer (at the time of your application).

Notes

This order will be reviewed by the Committee no later than 10 October 2024 or earlier if new evidence which is relevant to the Order becomes available after it was made.
