Allegation

As a registered Dietitian (DT24773) your fitness to practise is impaired by reason of misconduct and/or lack of competence. In that:  

  1. On or around 25 July 2019, you verbally provided incorrect dosage instructions to Service User A for their new medication, which if followed, could have resulted in Service User A experiencing hypoglycaemia. 
  1. On or on a date after 25 July 2019, you did not provide, in writing, correct and/or adequate dosage instructions to Service User A for the new medication as Service User A made multiple queries to seek clarification on the dosage. 
  1. On 26 July 2019 you advised and/or instructed a GP, in writing, to change Service User A’s medication, specifically insulin for diabetes, when you had no prescription training and without discussing and/or seeking authorisation to do so. 
  1. On 12 July 2019 you requested a GP to prescribe an additional medication to Service User B to support their diabetes when you had no prescription training and without discussing and/or seeking authorisation to do so. 
  1. On 17 January 2020 you commenced Service User C on a Very Low Calorie Diet (VLCD) and provided advice on medication reduction/s without liaising with a consultant or senior nurse from the Diabetes Care For You Team. 
  1. On 17 January 2020 you commenced Service User C on a Very Low Calorie Diet (VLCD) when you should not have done as Service C reported experiencing several episodes of hypoglycaemia following initiation of the diet. 
  1. On 13 January 2021 you commenced Service User D on a Very Low Calorie Diet (VLCD) and provided advice on medication reduction/s without liaising with a consultant or senior nurse from the Diabetes Care For You Team. 
  1. On 2 February 2021 you commenced Service User E on a Very Low Calorie Diet (VLCD) and provided advice on medication reduction/s, without liaising with a consultant or senior nurse from the Diabetes Care For You Team. 
  1. On 2 February 2021 you commenced Service User E on a Very Low Calorie Diet (VLCD) and left Service User F on a medication which could have caused a medical emergency and/or harm. 
  1. You did not screen for an eating disorder or for other medical conditions before commencing Very Low Calorie Diets (VLCDs).a).On 17 January 2020 for Service User Cb). On 13 January 2021 for Service User D c). On 2 February 2021 for Service User E 
  1. On 14 January 2021, you requested a GP to change Service User F’s medication, specifically rapid insulin, when you had no prescription training and without discussing and/or seeking authorisation to do so. 
  1. You did not complete and maintain accurate records of your discussions with Service Users during clinical appointments, in that:a).You did not keep clear notes, in patient records, of your discussions and your rationales for recommending, requesting and/or instructing medication changes for Service User A.b).You did not complete written care plans with advice about the dosages of the medications you suggested for Service User A.c).You did not complete follow up letters to the Service Users set out in  Schedule 1. 
  2. You did not complete follow up letters to the GPs of the Service Users set out in        Schedule 2.
  3.  In respect of particulars 1, 3-9 and 11, you worked beyond your scope of practice.
  4. The matters set out in particulars 1-13 above amount to misconduct and/or lack of competence.
  5. By reason of misconduct and/or lack of competence your fitness to practise is impaired.
