As a registered Physiotherapist (PH117287) your fitness to practise is impaired by reason of misconduct and/or lack of competence. In that:

1. On 15 November 2019, during a telephone assessment with Patient A:

a. You did not document a clinical hypothesis

b. You did not document an analysis in the record

c. You did not seek advice from the on-call clinician about Patient A’s
symptoms and/or make an emergency referral to the Emergency Department for Patient A.

d. you did not recognise and/or record the presence of Cauda Equina Syndrome and/or take adequate steps to refer Patient A for further treatment.

2. Between April and December 2019, you did not document analyses on the Subjective Objective Analysis Plan (SOAP) records for the Patients set out in Schedule A.

3. Between April and December 2019, you did not document aims/goals on the case records for the Patients set out in Schedule B.

4. Between April and December 2019, you did not complete adequate notes for the Patients set out in Schedule C in that you did not record sufficient clinical reasoning.

5. Between April and December 2019, you did not conduct and/or document screening measures undertaken for serious pathological conditions; for Patients set out in Schedule D.

6. During and/or subsequent to your assessment of Patient 5 on 4 November 2019, you did not document their history of cancer.

7. The matters set out in allegations 1 - 6 above constitute misconduct and/or lack of competence.

8. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
