The Interim Suspension Order made on 11 June 2021 is replaced by an Interim Conditions of Practice Order. The Registrar is directed to annotate the HCPC Register to show that from the date that this Order you, Mr Ikechukwu Ofoha, must comply with the following conditions of practice:

1. You must satisfactorily complete a return to work training course as required by the HCPC before undertaking any employment as a Radiographer.

2. You must place yourself and remain under the direct supervision of a workplace supervisor, being a Radiographer of Band 6 or above, registered by the HCPC, and supply details of your supervisor to the HCPC within 14 days of the supervisor being identified. You must attend upon that supervisor as required and follow their advice and recommendations.

3. You must not undertake any aspect of radiological imaging unless directly supervised by your workplace supervisor until you have been signed off by your supervisor to perform such aspect of radiological imaging without direct supervision.

4. You must meet with your supervisor on a weekly basis to consider any conduct and competence in relation to your practice that might arise.

5. You must allow your supervisor to provide information to the HCPC about your progress and any concerns relating to your conduct and/or competence no later than 5 days before any review of this order.

6. You must promptly inform the HCPC if you cease to be employed as a Radiographer or take up any other or further employment as a Radiographer.

7. You must promptly inform the HCPC if you become subject to any further disciplinary proceedings.

8. You must inform the following parties that your registration is subject to these conditions:

A. Any organisation or person employing or contracting with you to undertake professional work as a Radiographer;
B. Any agency you are registered with or apply to be registered with (at the time of application) as a Radiographer; and
C. Any prospective employer of you as a Radiographer (at the time of your application).

This Order will be reviewed by the Committee no later than 9 April 2024 or earlier if new evidence which is relevant to the Order becomes available after it was made.
