As a registered Radiographer (RA82444) your fitness to practise is impaired by reason of your lack of competence. In that:

1. Between February and April 2021, you did not demonstrate the skills and knowledge expected of an NHS Band 6 Therapeutic Radiographer. In that:

a) Between February and April 2021, you failed to demonstrate adequate basic anatomical knowledge, in that:

i. You were unable to verbalise soft tissue anatomy in detail.
ii. You were unable to verbalise bony anatomy in detail.
iii. You were unable to verbalise anatomical knowledge to the level expected of a Band 6 Radiographer.

b) On 29 March 2021, you carried out a pelvis and breast assessment unsatisfactorily, in that:

i. Your communication with the service user and staff was poor, as you spoke quietly and/or prompting was required on occasions and/or you verbalised incorrect information.
ii. Your clinical performance was not fluid and/or fully competent.
iii. You did not conduct the correct treatment checks.
iv. You did not complete the assessment in a time effective manner.

c) Between February and April 2021 you failed to complete one or more practical assessments to a satisfactory level.

d) By April 2021 you were unable to treat service users without receiving prompts.

e) By 8 April 2021 you had not demonstrated the following standards:

i. High levels of patient safety and care.
ii. Competencies for the role to be achieved in the induction period.
iii. Background knowledge and skills.

2. The matters set out in Particular 1 above constitute lack of competence.

3. By reason of your lack of competence your fitness to practice is impaired.
