Order

The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 12 months, you, Mr Shane Woolmore, must comply with the following conditions of practice:

1. You must confine your professional practice to the permanent role of EOC Triage Paramedic with South Western Ambulance Service NHS Foundation Trust (SWAST).

2. You must not carry out autonomous work as a sole Paramedic attending on patients, whether within the NHS or privately.

3. You must not carry out locum or agency work.

4. You must not carry out professional work in a private practice setting.

5. You must not carry out visits to a patient’s/service user’s private residence.

6. You must not be involved in the training of students or other health care professionals.

7. You must place yourself and remain under the direct supervision of your workplace supervisor or other qualified HCPC-registered Paramedic with relevant experience and who is trained to practise in the EOC setting, for at least one month and thereafter at their discretion.

8. You must arrange for a monthly audit of the following areas of practice by your workplace supervisor:

A. Record-keeping, including its accuracy and its clinical appropriateness;

B. Your role in triage and your interactions with patients/service users;

C. Clinical assessments;

D. Decision-making.

9. Your workplace supervisor must provide, for the next review of this Interim Order:

A. The results of the monthly audits;

B. A report with regard to your progress whilst under supervision.

10. You must inform the HCPC within seven days if you cease to be employed by your current employer.

11. You must inform the HCPC within seven days of becoming aware of:

A. any patient safety incident you are involved in;

B. any investigation started against you; and

C. any disciplinary proceedings taken against you.

12. You must inform any organisation or person employing or contracting with you to undertake professional work that your registration is subject to these conditions.

13. You must work with your workplace supervisor to formulate a Personal Development Plan designed to address any deficiencies identified in your practice.

14. You must meet with your workplace supervisor on a monthly basis to consider your progress towards achieving the aims set out in your Personal Development Plan.

15. You must maintain a reflective practice record detailing examples of your clinical assessments and decision-making, in particular addressing any deficiencies identified in your practice.

16. You must provide a copy of this reflective practice record to the HCPC before any review of this Interim Order.
