Finding
The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 18 months, you, Kelly Greener, must comply with the following conditions of practice:
1. You must place yourself and remain under the supervision of a workplace supervisor who must be a HCPC registered Speech and Language Therapist who has also achieved certified membership of the Royal College of Speech and Language Therapists.
2. You must supply details of your supervisor to the HCPC within 7 days of the Operative Date, or in advance of commencing any professional work requiring registration with the HCPC (if later) and the HCPC must have confirmed that they are acceptable, before you commence work.
3. You must ensure that your supervisor takes overall responsibility for the arrangements for your supervision. In discharging their responsibility to provide the registrant with appropriate supervision, it is expected the supervisor will:
i. Be present on-site and be available to give you advice and/or assistance at all times unless unable to do so and a nominated deputy is available;
ii. Meet with you formally, at least once a week, for case-based discussion and feedback. The frequency of these meetings can be reduced if agreed by the supervisor if you have met the agreed supervision objectives.
4. You must ensure, in the event of your supervisor’s unavailability that a named deputy or deputies nominated by your supervisor must be available to give advice and/or assistance at all times and provide feedback to your supervisor about your practice. Any deputy must be a HCPC registered Speech and Language Therapist who has also achieved certified membership of the Royal College of Speech and Language Therapists. Any named deputy or deputies may meet with you on your supervisor’s behalf provided they make a written record and update your supervisor about the conversation. This should only happen in limited circumstances where your supervisor is unable to meet with you themselves.
5. You must attend upon your supervisor as required and follow their advice and recommendations.
6. You must inform the HCPC within seven days if you take up any other or further professional work.
7. You must inform the HCPC within seven days if you take up work requiring registration with a professional body outside the United Kingdom.
8. You must inform the HCPC within seven days of becoming aware of:
A. any patient safety incident you are involved in;
B. any investigation started against you; and
C. any disciplinary proceedings taken against you.
9. You must inform the following parties that your registration is subject to these conditions:
A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
C. any prospective employer for professional work (at the time of your application);
D. any organisation through which you are undertaking professional training;
F. the occupational health provider for your current employer.
12. You must allow the HCPC to share, as necessary, details about your performance, compliance with, and/or progress under these conditions with:
A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
C. any prospective employer for professional work (at the time of your application);
