Order: The Registrar is directed to impose an Interim Conditions of Practice Order for a period of 18 months as follows:

1) You must confine your professional practice to one single employer.

2) You must place yourself and remain under the indirect supervision of a workplace supervisor, registered with the HCPC, and supply details of your supervisor to the HCPC with 14 days.

3) You must meet with that supervisor every two weeks to discuss your understanding of professional boundaries and follow their advice and recommendations.

4) You must ensure that your supervisor sends a progress report to the HCPC at least 14 days before any HCPC review of this Order. This report should assess your professional performance and comment on your ability to maintain appropriate professional conduct with work colleagues.

5) You must inform the HCPC within 14 days if your employment with the Trust is terminated, whether by you or by your employer.

6) You must notify the HCPC within 14 days of any other appointment as an Operating Department Practitioner that you accept within the UK or elsewhere and provide the HCPC with the contact details of your employer.

7) You must inform the following parties that your registration is subject to the interim conditions in this order:
A. any organisation or person employing or contracting with you to undertake professional work as an Operating Department Practitioner;
B. any agency you are registered with or apply to be registered with (at the time of application); and
C. any prospective employer (at the time of your application).

This order will be reviewed by the Committee no later than 21 May 2025 or earlier if new evidence which is relevant to the Order becomes available after it was made.
