Allegation

As a registered Radiographer (RA084601) your fitness to practise is impaired by reason of your misconduct and/or lack of competence. In that:

  1. On 10 December 2021 you performed a CT chest scan without contrast instead of the required CT chest scan with contrast on Service User A; and/or

2. On 20 December 2021 you performed a CT chest scan instead of the requested CT head scan on Service User B; and/or

3. On 12 March 2022 you assigned a CT scan to an incorrect locum radiologist for reporting; and/or

4.On 31 March 2022 you performed a scan under an incorrect name on Service User C; and/or

5. On 08 April 2022 you did not allow sufficient time for Colleague A to leave the proximity of the CT scanner before initiating the scan; and/or

6. On 12 May 2022 you did not follow workplace procedure of contacting a radiologist for support following four failed cannulation attempts and made further attempts yourself; and/or

7. On 12 May 2022 you did not communicate professionally with Colleagues B and C while cannulating a patient in that you:

a) Disputed Colleague B’s clinical decision in front of a patient; and/or

b) Ignored Colleague B’s reminder of protocol requiring radiologist support following four failed cannulation attempts

8. On 23 June 2022 you performed a CT scan on Service User D without contrast; and/or

9. On 24 June 2022 you assigned a scan on Service User E to an incorrect patient;

10. On 23 June 2022 you created a DATIX report in which you attributed the incident at particular 8 to machine failure when this was not the case;

11. On 6 September 2022 you informed the HCPC that you had told Your World Healthcare that you were subject to a fitness to practise investigation when this was not the case.

12. Your conduct in relation to Particulars 10,and/or 11 was dishonest.

13. The matters set out in Particulars 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11 and/or 12 constitute misconduct and/or a lack of competence.

14. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
