Order
ORDER: The Conditions of Practice Order imposed on 7 June 2024 and amended on 19 September 2024 is confirmed as follows:
1. You must register with and remain under the care of a general practitioner [redacted], inform them that you are subject to these conditions and authorise that person to provide the HCPC with information about your health and any treatment you are receiving.
2. You must provide the HCPC with the contact details of your general practitioner [redacted] within seven days of the Operative Date or within seven days of this person changing.
3. You must be reviewed by your general practitioner [redacted] on a regular basis and no less than once every six weeks and the purpose of these reviews is to monitor your stability and compliance with medications.
4. You must provide a report from your general practitioner [redacted] if requested by the HCPC.
5. You must work in a part-time role, whether as a locum, or employed directly, unless and until your general practitioner [redacted] states you are fit for full-time work.
6. You must keep your professional commitments under review and limit your professional practice in accordance with the advice of your general practitioner [redacted].
7. You must cease practising immediately if you are advised to do so by your general practitioner [redacted]. You must inform the HCPC within seven days of receiving this advice.
8. You must place yourself and remain under the indirect supervision of a workplace line manager registered by the HCPC or other appropriate statutory regulator and supply details of your manager to the HCPC within 4 weeks of commencing employment. You must attend upon that manager as required and follow their advice and recommendations.
9. You must obtain a report from your workplace line manager dealing with how you are managing your workload, to be submitted to the HCPC 14 days before each review hearing.
10. You must inform the following parties that your registration is subject to these conditions:
A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
C. any prospective employer for professional work (at the time of your application);
D. the occupational health provider for any employer/contracting body.
Notes
This order will be reviewed by the Committee no later than 1 July 2025 or earlier if new evidence which is relevant to the Order becomes available after it was made.
