Allegation

As a registered Occupational Therapist (OT72541) your fitness to practise is impaired by reason of lack of competence. In that: 
 
1. During your employment with NHS Lothian between January and June 2017, you did not meet the competencies of a Band 5 Occupational Therapist, in that:  
 
 
a. You were unable to carry an appropriate number of cases as expected of a Band 5 Occupational Therapist; 
 
b. You were unable to manage your allocated caseload without daily supervision; 
 
c. You were unable to carry out and/or prioritise required assessments and/or interventions in an adequate and/or independent and/or timely manner, for:

d. You did not record patient notes in an adequate and/or timely manner for:

2. On 1 October 2017, you submitted a de-registration form to the HCPC Registrations department, in which you stated you were unaware of any investigations against you, which was incorrect.
 
3. The matters set out at allegation 1 - 2 above amount to lack of competence. 
 
 
4. By reason of your lack of competence your fitness to practise is impaired.

ORDER: The Panel granted the voluntary removal agreement application. The Registrar is directed to remove the name of Miss Klara Tjader Engstrom from the Register with immediate effect.
