Order
ORDER: The Registrar is directed to annotate the Register to show that, for a period of 12 months from the date this Order comes into effect, you, Mr Alexander Bartok, must comply with the following conditions of practice:-
1. Except in life threatening emergencies, you must not be involved in the direct (face-to-face) treatment of female patients, without a chaperone, who does not need to be registered with the HCPC, being present.
2. You must maintain a record of every case where you have treated a female patient face to face, in each case signed by the chaperone.
3. You must provide a copy of these records to the HCPC 14 days before the date of the next review hearing, or alternatively, confirm that there have been no such cases during that period. You must make these records available for inspection at all reasonable times by any person authorised to act on behalf of the HCPC.
4. If working in an ambulance you must not work solely alongside a female colleague or tutor a female student.
5. You must maintain a record of every shift where you work in an ambulance confirming compliance with condition 4, in each case signed by your assigned Team Leader.
6. You must provide a copy of these records to the HCPC 14 days before the date of the next review hearing or, alternatively, confirm that there have been no such cases during that period. You must make these records available for inspection at all reasonable times by any person authorised to act on behalf of the HCPC.
7. You must promptly inform the HCPC if you take up any other or further employment.
8. You must inform the following parties that your registration is subject to these conditions:
A. any organisation employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with for professional work (at the time of application); and
C. any prospective employer for professional work (at the time of application).
Notes
This Order will be reviewed by the Committee no later than 19 August 2025 or earlier if new evidence which is relevant to the Order becomes available after it was made.
