The Registrar is directed to place the following Conditions of Practice for a period of 18 months:

1) You must place yourself and remain under the supervision of a workplace supervisor who is a Clinical Scientist more senior to yourself and is registered by the HCPC or other appropriate statutory regulator.

a. You must supply details of your supervisor to the HCPC within 7 days of returning to work.

b. You must have regular meetings with and attend upon that supervisor as required and follow their advice and recommendations.

c. You must work with your supervisor to formulate a Personal Development Plan designed to address the deficiencies in your performance.

d. Within one month of your return to work you must forward a copy of your Personal Development Plan to the HCPC.

e. You must allow your supervisor to provide information to the HCPC about your progress towards achieving the aims set out in your Personal Development Plan.

f. You must forward to the HCPC any formal feedback from your supervisor within 7 days of receiving it.

2) You must inform the HCPC within seven days if you cease to be employed by your current employer.

3) You must inform the HCPC within seven days if you take up any other or further professional work.

4) You must inform the HCPC of the outcome of any internal investigation that is carried out by your Employer within 7 days of being notified of the outcome.

5) You must confine your professional practice to one NHS Employer under the supervision requirements set out at Condition 1 above.

6) You must not carry out any locum or agency work.

7) You must not carry out professional work in a private practice setting.
