The Interim Conditions of Practice Order made on 21 August 2024 is confirmed. You, Mr Chukwudi O Ogbonna, must comply with the following conditions of practice:
1. Only be employed by one substantive employer at a time for the purposes of providing professional services as a Radiographer.

2. Only provide professional services as a Radiographer when directly supervised by a HCPC registered Radiographer.

3. Not undertake any ultrasound scans unless directly supervised by a HCPC registered radiographer who is a qualified sonographer, until you have been signed off as competent by your workplace supervisor in the relevant area of ultrasound.

4. Place yourself and remain under the supervision of a HCPC registered Radiographer. You must:
a. Meet with your supervisor at least once each week;
b. follow your supervisor’s advice and recommendations.
c. allow your supervisor to provide information to the HCPC about your competence at least seven days before any review of this order;
d. supply details of your supervisor to the HCPC within 14 days of commencing employment;

5. Inform the HCPC within seven days if you cease to be employed as a Radiographer.

6. Inform the HCPC within seven days if you take up any professional work (paid or unpaid) as a Radiographer.

7. Inform the HCPC within seven days of becoming aware of:
a. any patient safety incident you are involved in;
b. any investigation started against you; and
c. any disciplinary proceedings taken against you.

8. Inform the following parties that your registration is subject to these conditions:
a. any organisation or person employing or contracting with you to undertake professional work as a Radiographer;
b. any prospective employer for professional work as a Radiographer (at the time of your application);
c. any supervisor required by these conditions.

This order will be reviewed by the Committee no later than 18 June 2025 or earlier if new evidence which is relevant to the order becomes available after it was made.
