As a registered Operating Department Practitioner (ODP12110) your fitness to practise is impaired by reason of misconduct and/or a health condition. In that:
1. On 02 November 2021, having given evidence to a Medical Practitioners Tribunal Service hearing that you had calendar entries on your mobile phone that were relevant to your allegations, you created new calendar entries on your mobile phone in that:
a. You created the entry “[Dr 1] AA” for the date of 07 March 2017.
b. You created the entry “[Dr 1]” for the date of 24 May 2018
c. You created the entry “[Dr 1]” for the date of 26 October 2018.
2. Having created new calendar entries, you forwarded screenshots of them to the General Medical Council without disclosing that you had created those entries after giving your evidence on 2 November 2021.
3. Your conduct in relation to particulars 1 and 2 above was dishonest, in that you knowingly concealed material information about the creation of the calendar entries from the General Medical Council.
4. You have a physical and/or mental health condition as set out in Schedule A.
5. The matters set out in particular 1, 2, and 3 constitute misconduct.
6. By reason of your misconduct and/or health your fitness to practise is impaired.
