Allegation
As a registered Operating Department Practitioner (ODP12110) your fitness to practise is impaired by reason of misconduct and/or a health condition. In that:
1. On 02 November 2021, you falsified calendar entries on your mobile phone to support your allegations in a General Medical Council Tribunal Service hearing, in that:
a. You created the entry “Mk AA” for the date of 07 March 2017.
b. You created the entry “Mk” for the date of 24 May 2018.
c. You created the entry “Mk” for the date of 26 October 2018.
2. Your conduct in relation to particular 1 above was dishonest.
3. You have a physical and/or mental health condition as set out in Schedule A.
4. The matters set out in particular 1 and 2 constitute misconduct.
5. By reason of your misconduct and/or health your fitness to practise is impaired. Schedule referred to the Conduct and Competence Committee
