Allegation
Joined Allegation
As a registered Paramedic (PA47587):
1. Prior to commencing work with Acute Ambulance & Medical Services (AAMS) in or around June 2019, you informed them:
a. you had been a registered paramedic for five years before coming off the HCPC register for health reasons, when this was not the case;
b. you attended major incidents as a registered paramedic, such as the Manchester bombings in 2017, when this was not the case.
2. You included inaccurate information within a Curriculum Vitae uploaded to a recruitment website, in that you stated you worked as a paramedic for the West Midlands Ambulance Service between 2015 and 2018, when that was not the case.
3. Between 21 June 2019 and 21 September 2019, you placed service users and/or colleagues and/or members of the public at risk of harm in that, on one or more occasions you:
a. completed consecutive shifts without having had adequate rest;
b. drove a company vehicle after working consecutive shifts, without having had adequate rest;
c. drove a company vehicle above the speed limit without adequate clinical justification for doing so.
4. Since July 2019, you have not returned a Mangar Elk lifting cushion belonging to AAMS, despite AAMS making multiple requests for you to do so.
5. On 21 September 2019 you sent a WhatsApp message to Colleague 11 in which you wrote “I spoke to the staff at 08:15ish and they explained how to drive down”, when this was not the case.
6. Between 05 August 2019 and 30 November 2019, you billed for an additional 173 hours of office work and/or billed for hours worked when you were on leave.
7. On or around 04 January 2020, during a telephone call with Colleague 14, you:
a. disclosed confidential information, in that you informed Colleague 14 that Colleague 11 was under investigation;
b. stated that Colleague 11 ‘had no medical training’, or words to that effect.
8. Between 01 October 2019 and 03 December 2019, you administered and/or supplied the following medications when they were not clinically indicated and/or it was inappropriate to do so:
a. Diazemuls outside the conditions of use in relation to Service User A
b. Morphine outside the conditions of use in relation to the service users set out in Schedule A,
c. Salbutamol outside the conditions of use in relation to the service users set out in Schedule B,
d. Ipratropium outside the conditions of use in relation to the service users set out in Schedule C,
e. Chlorphenamine outside the conditions of use in relation to the service users set out in Schedule D,
f. Hydrocortisone outside the conditions of use in relation to the service users set out in Schedule E,
g. Ibuprofen outside the conditions of use in relation to Service User B,
h. Intravenous Sodium Chloride outside the conditions of use in relation to the service users set out in Schedule F,
i. Intravenous Paracetamol outside the conditions of use in relation to the service users set out in Schedule G,
j. Intravenous Glucose outside the conditions of use in relation to Service User K,
k. Codeine outside the conditions of use in relation to Service User A.
9. You did not record a pain score prior to and subsequent to each administration of Morphine to Service User F
10.You inappropriately delayed administering pain relief to Service User I
11.Between 01 October 2019 and 03 December 2019, you did not ensure the safety of patients, in that you did not contact the South Central Ambulance Service’s Urgent Care Desk to seek clinical validation for the safe discharge of non-conveyed patients and/or you did not record your contact with the Urgent Care Desk on the Patient Record Forms (PRFs)
for one or more of the service users set out in Schedule H.
12.Between 01 October 2019 and 03 December 2019, you did not provide adequate and/or specific worsening advice and/or record the provision
of such advice on the PRFs to one or more of the non-conveyed service users set out in Schedule I.
13.You did not make a formal referral and/or record the making of such a referral to the GP of one or more of the Service Users in Schedule J.
14.Between around January 2020 to October 2020, you did not adequately notify Medic Now of the ongoing HCPC investigation and / or interim conditions of practice order
15.You did not communicate appropriately with Colleague 2, who was a witness to the investigation carried out by Elite EMS, in that;
a. In or around December 2019, you told Colleague 2 that you would ‘throw him under the bus’, or words to that effect,
b. On or around 12 March 2020, you told Colleague 2 that he would be ‘bought up on criminal charges’, or words to that effect,
c. On or around 25 June 2020, you told Colleague 2 that you would be ‘suing him for perjury’, or words to that effect.
16.In respect of the matters set out at particular 8 above, you have worked beyond your scope of practice.
17.Your conduct in relation to particulars 1a-1b, 2, 5, 6, 7b and 14 above was dishonest.
18.The matters set out in particulars 1-17 above constitute misconduct.
19.By reason of your misconduct, your fitness to practise is impaired
