Order


ORDER: The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 18 months, you, Mr Ugochukwu Obiezu, must comply with the following conditions of practice:

1. You must only be employed by one substantive employer at a time which at the moment is Birmingham Community Healthcare for the purposes of providing professional services as a Physiotherapist. The employer shall not be an employment agency.

2. You must place yourself and remain under the direct supervision of a Band 7 Physiotherapist registered by the HCPC and supply details of your supervisor to the HCPC within 14 days of the Operative Date. You must attend upon that supervisor as required and follow their advice and recommendations.

3. You must arrange for weekly supervisions to consider your progress towards achieving the aims set out in your Personal Development Plan.

4. You must obtain from your supervisor a report describing your clinical performance and your engagement with that supervisor, and you must submit that report to the HCPC two weeks before the date of the review of this Interim Conditions of Practice order.

5. You must inform the HCPC within seven days if you cease to be employed by Birmingham Community Healthcare.

6. You must inform the HCPC within 7 days if you take up professional work in a different organisation to Birmingham Community Healthcare.

7. You must inform the HCPC within seven days of becoming aware of:

a. any patient safety incident you are involved in;

b. any investigation started against you;

c. and any disciplinary proceedings taken against you.

8. You must inform the following parties that your registration is subject to these conditions:

a. any organisation or person employing or contracting with you to undertake professional work, such notification to be made as soon as possible;

b. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);

c. any prospective employer for professional work (at the time of your application).

9. You must allow the HCPC to share, as necessary, details about your performance, compliance with, and/or progress under these conditions with:

a. any organisation or person employing or contracting with you to undertake professional work;

b. any prospective employer for professional work (at the time of your application).

Notes

This order will be reviewed by the Committee no later than 23 March 2026 or earlier if new evidence which is relevant to the Order becomes available after it was made.
