ORDER: The following Interim Conditions of Practice Order is confirmed.

1. You must confine your professional practice to activities which do not require patient contact.
2. You must register with and remain under the care of a General Practitioner and Consultant Psychiatrist and inform them that you are subject to these conditions.
3. You must provide the HCPC with the contact details of your General Practitioner and Consultant Psychiatrist within seven days of the date of this Order.
4. You must provide the HCPC with the contact details of your Community Mental Health Team within seven days of the date of this Order.
5. You must authorise your General Practitioner and Consultant Psychiatrist to provide the HCPC with information about your health and any treatment you are receiving.
6. You must authorise your Community Mental Health Team keyworker to provide information to the HCPC.
7. You must arrange for the provision of the HCPC Health Reference Form reports from your Consultant Psychiatrist to the HCPC every 3 months beginning on the date of this Order and covering: your current condition; compliance with treatment and the management of your condition; current medication; and prognosis.
8. You must keep your professional commitments under review and limit your professional practice in accordance with the advice of your General Practitioner and Consultant Psychiatrist.
9. You must cease practising immediately if you are advised to do so by your General Practitioner and/or Consultant Psychiatrist. You must inform the HCPC within seven days of receiving this advice.
10. You must inform the HCPC within seven days if you take up any other or further professional work.
11. You must inform the HCPC within seven days if you take up work requiring registration with a professional body outside the United Kingdom and
12. You must inform the HCPC within seven days of returning to practice in the United Kingdom.
13. You must inform the following parties that your registration is subject to these conditions:
A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
C. any prospective employer for professional work (at the time of your application);
D. any organisation through which you are undertaking professional training.

This order will be reviewed by the Committee no later than 1 August 2026 or earlier if new evidence which is relevant to the Order becomes available after it was made.
