1. On 24 February 2023 you did not obtain Service User A's informed 
consent before carrying out a procedure which involved nail trimming 
with a scalpel.  

2. On 24 February 2023 you did not maintain accurate and/or complete records in that you:  

a. Did not document Service User A’s medical and/or medications history.  

b. Did not document Service User A's medical concern.
  
c. Did not document that Service User A presented with an ingrowing toenail.  

d. Did not document an accidental incision made to Service User A's toe

e. Did not document worsening advice given to Service User A in relation to the incision to their toe.
  
3. On 21 April 2023, you did not maintain accurate and/or complete records in that you did not record that you had advised Service User A that they may require nail surgery.  

4. On 18 May 2023 you did not maintain and complete records for Service User A in that you did not record details of their appointment that took place on 18 May 2023.  

5. On 16 June 2023, you advised Service User A that they had attended an appointment on 24 February 2023 whilst presenting with an ingrowing toenail.  

6. Your actions at particular 5 were dishonest because you sought to misrepresent the reason for Service User A’s initial appointment.  

7. The matters set out in particulars 1-6 above constitute misconduct.  

8. By reason of the matters set out above, your fitness to practise is impaired by reason of misconduct.
