As a registered Physiotherapist (PH125470) your fitness to practice is impaired by reason of misconduct. In that:  

  1. In or around 10 August 2023, you submitted a forged cultural competence course completion certificate to support your application for registration with the Physiotherapy Board of New Zealand (“the Board”).  
  1. In correspondence between you and the Board between 12 August 2023 and 17 August 2023, you gave conflicting reasons for submitting the forged certificate referred to at particular 1 above.  
  1. Your conduct in relation to particulars 1 and 2 above was dishonest.  
  1. The matters set out in particulars 1, 2 and 3 above constitute misconduct.  
  1. By reason of your misconduct your fitness to practise is impaired.
