As a registered Operating Department Practitioner (ODP11945):

1. On 26 July 2023, you were convicted at Sheffield Magistrates’ Court of the following offence, “on 09/07/2023 when suspected of having been in charge of a vehicle and having been required to provide a specimen or specimens of breath for analysis by means of a device of a type approved by the secretary of state pursuant to section 7 of the Road traffic Act 1988 in the course of an investigation into whether you had committed an offence under section 3A, 4, 5, or 5A thereof, failed, without reasonable excuse, to do so”.

2. On 20 November 2023, you attended work whilst under the influence of alcohol.

3. On 15 April 2024, you were convicted at Sheffield Magistrates Court of the following offences:

a. On 20 November 2023, you drove a motor vehicle after consuming so much alcohol that the proportion of it in your breath, namely 128 microgrammes of alcohol in 100 millilitres of breath, exceeded the prescribed limit.

b. On 27 March 2024, you drove a motor vehicle, after consuming so much alcohol that the proportion of it in your breath, namely 142 microgrammes of alcohol in 100 millilitres of breath, exceeded the prescribed limit.

4. You have a physical and/or mental health condition as set out in Schedule A.

5. The matters set out in particular 2 above constitutes misconduct.

6. By reason of the matters set out above, your fitness to practise is impaired by reason of conviction, and/or misconduct and/or health.
